May be tough to find , but look for a few North to South die scratches to the east of the eye. A couple from the brow to the bridge of the nose. Those being in protected areas may have suvived. If you have those I'd say you have it.
I am looking at Billy's book and your specimen does not have the vertical die scratches he refers to and which are pictured in his book on page 37. Your specimen does show a couple smaller ones in the same general area but not they are not the ones he has pictured in the book. So , I would have to say it is not a CDDO-004. But you could send it off to Billy.
